Struggling with a Mysterious Illness: Seeking Guidance (33F)

For the past seven months, I've experienced a persistent low-grade fever, fluctuating between 99.4 and 101.5 degrees daily. Recently, I noticed significant swelling in my joints for the first time, with an unusually large bump forming on the outside of my wrist. The discomfort and heat in my hands are becoming increasingly severe. Yesterday, I also developed a burning sensation accompanied by a rash that resembles scratch marks; however, it’s not caused by scratching. If that were the case, it would have diminished or disappeared by now, rather than causing the ongoing burning feelings I am experiencing. This rash is spreading to my other hand, arm, shoulder, and various areas on my torso, although in those regions, the marks are more dispersed. Both sides of my ribs feel inflamed and painfully swollen, with lymph nodes around my torso becoming tender, particularly on the left side. There's a bruised sensation along my spine in the center, and deep breathing has become challenging when the inflammation flares up. During these flare-ups, my hands and nails turn a reddish color, and since December, my nails have remained white with red tips. I'm struggling with overall muscle weakness, especially noticeable during physical activity, and there's a marked loss of strength in my hands and arms, as well as difficulty rising from a seated position. I have recently received a diagnosis of Postural Orthostatic Tachycardia Syndrome (POTS). A subsequent test from a different lab was negative, and I’ve consistently been told that my TPO and Tg antibodies are rising, currently both in the 250s, but endocrinology insists that they aren’t the cause of my symptoms since my thyroid function has remained stable, leading to a diagnosis of Hashimoto’s, which is not yet active. My SED rate and CRP levels have been persistently high, fluctuating considerably (ESR between 64 and 35, CRP between 22.7 and 8.7), with an IL-8 level of 98.7 and a C4a level of 4719 recorded in February. Other tests show normal C3, C4, borderline low ferritin, elevated folate levels, and slightly high RBC counts, alongside a D-Dimer result of 650 with a normal INR. I carry the BRCA1 mutation.
